Anyone over 18 volunteering with children or students, caregiver relief or anyone in a leadership position.
Before Volunteering and Getting Fingerprinted
Make sure you have completed the Volunteer Application and reviewed it with your Ministry Leader. If you haven’t already done so, please contact the church (Gwyn Smith, gwyn@thechurchatrb.org / 858.592.2434 x338)
The Fingerprinting Process (for volunteers and leaders 18 years and older)
In order to help CRB continue to be above reproach in this area, please complete the entire Fingerprinting Process within 2 weeks (from initial contact to return of paperwork to CRB)
1. Download and print out 1 copy of the Request for Live Scan Service Form
2. Complete the middle section of the form: (Name of Applicant through Social Security Number.)
3. Make Fingerprinting Appointment: Call a Live Scan location and make an appointment.

4. Contact the church (Gwyn Smith, gwyn@thechurchatrb.org / 858.592.2434 x338) with the date of your fingerprinting appointment.
5. Go to Your Fingerprinting Appointment: Please bring the following with you to your appointment:
Your completed Request for Live Scan Service form.
Valid photo identification: a California Driver’s License or any state ID issued by the DMV.
Fee (Usually $20-$23 cash/check made payable to the Live Scan location of your appointment.
After Being Fingerprinted
Bring back the completed Live Scan Service form to the church office (Gwyn).
Should you wish to be reimbursed for the fingerprinting fee, please complete this CRB Reimbursement Form, attach your receipt to the form and return it to Gwyn Smith or the Admin Team. Reimbursements typically take 1-2 weeks.
